Evidence of identity (EOI) for Driver Licence and Vehicle Registration Transactions

Description

 Evidence of Identity (EOI) is the first requirement for a new customer being recorded on the Motor Registry System (MRS).  EOI establishes a person's identity including their age, residential address and their existence within the community.

 

You must establish EOI when conducting Registration and Licensing transactions.

 

EOI is also used to establish the legal entity or status of a company or incorporated association (ie a body corporate). 

 

All EOI documents must be in English, or accompanied by an English translation from an accredited translator.

 

Evidence of Identity - Individual

 

You can establish EOI by providing an Australian or New Zealand photo driver licence with your correct name and address details on it.  The licence can be expired up to two (2) years.

 

If the name and/or address details are not correct, then you must also provide Evidence of Residential Address and/or Evidence of Change of Name documents. 

 

If you cannot provide the above, you must provide full EOI by providing at least three (3) documents, either:

·         One document from Category A plus two documents from Category B, or

·         Two documents from Category A plus one document from Category B

 

At least one document from either category must show your date of birth and current Tasmanian residential address. If the documents you provide dont show a current Tasmanian residential address, you will need to provide a fourth document less than six (6) months old from the Evidence of Residential Address category.  All documents must be original and current (photocopies or certified copies are not acceptable).

 

If your name is different than what is shown on your Category A documents, you must also provide at least one original Evidence of Change of Name document.

 

All EOI documents must be in English, or accompanied by an English translation from an accredited translator.

 

Category A Documents

 

All documents must be original and current (photocopies or certified copies are not acceptable).

 

·         Birth Certificate (not an Extract or commemorative certificate)

·         Citizenship papers

·         Australian Passport

·         Overseas Passport / Visa

·         Driver Licence (can be expired up to 2 years)

·         Firearm Licence

·         Tasmanian Government Personal Information Card (displaying full name)

·        Department of Immigration & Citizenship issued travel documents, including a    visa

·        Department of Immigration & Citizenship Certificate of Evidence of Resident Status

 

Category B Documents

 

All documents must be original and current.

 

·         Medicare Card

·         Plastic Bank or Credit Card, with signature

·         Tertiary Student Identity Card

·         Department of Veterans Affairs / Centrelink Pensioner Concession Card

·         Births, Deaths & Marriages Change of Name / Marriage Certificate or Deed Poll

 

Evidence of Residential Address

 

If none of your EOI documents show your current Tasmanian residential address, you must provide either of the following original documents (less than 6 months old) to establish your current residential address:

·         Bank Statement

·         Utility Account (with evidence of payment)

 

Evidence of Garage Address

 

All vehicles registered in Tasmania must have a Tasmanian garage address, which is:

·         Where the vehicle is normally kept (light vehicles with a Gross Vehicle Mass of 4.5 tonnes or less), or

·         Principal depot or base of operations of the vehicle (heavy vehicle with a Gross Vehicle Mass greater than 4.5 tonnes)

 

Where your residential and garage addresses are identical, no additional documents are required.  If the garage address is different to your residential address, you must provide one of the following original documents to establish your vehicles garage address:

  • Certificate of Title
  • Lease or Rental agreement
  • Taxation Assessment Notice in relation to the premises (last or current year)
  • Council Rates Notice

·     Utility account less than six months old relating to the premises (gas, electricity, telephone, water etc)

 

Evidence of Change of Name

 

If your EOI documents dont show your new name, you must provide one of the following documents.  All documents must be original and current:

·         Marriage Certificate issued by Births, Deaths & Marriages (or interstate/overseas BDM equivalent) - Church or Celebrant issued certificates are not acceptable.

·         Divorce Paper indicating the name being reverted to, as issued by a Court

·         Change of Name Registration issued by Births, Deaths & Marriages or Deed Poll

·         Guardianship Order issued by the relevant authority

·         Adoption Paper issued by a Court

 

Non-Standard EOI

 

Some people may be genuinely unable to provide the required EOI documents.  Genuine inability doesnt mean inconvenience or delay to a person, or difficulty or expense in obtaining a required document.

 

If you are genuinely unable to meet the full EOI requirements, you must provide at least one document from Category A plus two or more non-standard EOI documents.  At least one non-standard EOI document must display your current Tasmanian residential address.  All documents must be original and current.

Requirements for a Body Corporate

 

A body corporate is a legal entity that can either be a company or an incorporated association.

 

The EOI requirements for a body corporate are based on the legal status information provided in the incorporation registration certificate (ie Certificate of Registration or other documents as detailed below).  Only current documents (less than six months old) are acceptable, and they must show the effective date of registration and/or the registered period of incorporation.

 

If the name and/or address details are not correct, then Evidence of Premise Address and/or Evidence of Change of Name documents must be provided.  The documents must be less than six (6) months old.

 

All EOI documents must be in English or accompanied by an English translation from an accredited translator.

 

Full Evidence of Identity - Body Corporate

 

To establish EOI for a Body Corporate (a company or incorporated association), you must provide at least one of the following documents:

·         Certificate of Registration (with ACN)

·         Certificate of Registration of Change of Name (with ACN)

·         Extract of Company (with ACN)

·         Certificate of Registration of Foreign Company (with ARBN)

·         Certificate of Incorporation

·         Written authorisation on letterhead by the issuing authority, stating the Act under which established

 

Photocopies of incorporation registration documents are acceptable.  The document/s must show the relevant:

·         Issuing authority (ie ASIC), or letterhead of the issuing authority

·         Registered/Incorporated Name of Body Corporate

·         Registration number (ie ACN, ARBN etc)

·         Effective date of incorporation, or incorporation registration period

 

Evidence of Premise Address

 

If none of the body corporate EOI documents show the current premise address, you must provide one of the following documents to establish the street address where the body corporate is located or operates from:

·         Contract of Purchase

·         Current Lease or Rental Agreement

·         Current Council Rates Notice

·         Current Land Tax Valuation Notice

·         Australian Taxation Office Assessment (last or current financial year)

 

Evidence of Garage Address

 

All vehicles registered in Tasmania must have a Tasmanian garage address, which is:

·         Where the vehicle is normally kept (light vehicles with a Gross Vehicle Mass of 4.5 tonnes or less), or

·         Principal depot or base of operations of the vehicle (heavy vehicle with a Gross  Vehicle Mass greater than 4.5 tonnes)

Evidence of Change of Name

 

If the correct name of a body corporate is not shown on the incorporation registration document, you must provide an additional document from the issuing authority (eg ASIC, Business Affairs etc) showing the current registered name (eg for a company a Certificate of Registration of Change of Name, including ACN, as issued by ASIC etc).
